1. Fill out application details below. For questions that are not applicable, please write NA.
2. Submit your accomplished form to the cashier area together with the following requirements:
a.) Perfumer’s Club fee of Php500
b.) (1) Valid Government Issued ID
3. Perfumer’s Club Loyalty Card will be issued to you once requirements are submitted and verified.
4. You can instantly use your new PERFUMER’S CLUB LOYALTY CARD once issued. Thank you and enjoy shopping with us!
IMPORTANT: For client security, we advise that you provide your full name and other verifying details. In case of lost or damaged card. The use of alias or nickname is highly discouraged.
